If you already have an active (and paid up) account, i think there is an activation fee for a new sim, but many times they will waive it if you complain.

If you go into an AT&T store, you could see if they'll give you a new sim for free and activate it right there. If you do it online, i don't think you'll have much luck. That said, if you have a twitter account, try @ATTCares and see if they can help you that way. They were very nice last I used that service.
